Privacy Policy

The State of California Information Practices Act of 1977 requires the University to provide this information to applicants for admission who are asked to supply information about themselves. Maintenance of the information is authorized by university policy.
The University requests the personal information on the application form to process an application for admission to COSMOS and financial assistance. Response to the questions is mandatory to complete the application unless a question is marked as optional. If an applicant does not supply all mandatory information, the processing of the application may be prevented from moving forward.
The University requires personal information about the applicant to process the application, including:
• Full legal name
• Applicant’s high school level
• Date of birth
• Address, telephone number, and e-mail address
• Campus and cluster choices
• Academic history, including high schools and colleges attended, courses are taken, grades, and standardized test scores
• Personal essay response(s). Other information, such as parents’ occupations and family income, is required only if an applicant is applying for financial assistance. Any document mailed to Statewide must have all Social Security Numbers removed or redacted.
The COSMOS Statewide office maintains submitted information and documentation to process the application. The University may be required to share personal information submitted to the state and federal governments or to respond to other lawful requests; to protect our rights and property and those of our agents, customers, and others, including to enforce our agreements and policies; and in an emergency, to protect the safety of our students, faculty, and staff or any other third party.
Aggregated information that will render the student unidentifiable as an individual may be included on, e.g., required public statistical analysis reports, identifying general statistics for COSMOS applicants and attendees.
Your Rights
Applicants and their parents and/or legal guardians (for applicants under the age of 18) have the right to review the applicant’s personal information, request to have it deleted, and refuse to allow further collection or use of the applicant’s information, to the extent permitted by law.
With applicant authorization, collected information or personal references may be disclosed by COSMOS to specific third parties of the applicant’s choice.
